1. Eligibility and certification

Certification is complete before billing; the record supports homebound status and the need for qualifying skilled services.The face-to-face encounter date and supporting clinical note or discharge summary are present.The practitioner record and agency chart tell a consistent clinical story.

2. Plan of care and orders

The plan covers ordered disciplines, services, frequency, duration, diagnoses, goals, and precautions that apply.Required signatures and dates are present before billing; verbal orders are recorded and countersigned as required.Each material change has a corresponding order and clear chronology.

3. Visit documentation

Each visit has a complete, signed, and dated note from the correct discipline.Documented service and teaching are consistent with the active plan and orders.Missed, shortened, or changed visits have an explanation and required follow-up.

4. EVV and authorization

When Medicaid EVV applies: service type, recipient, date, location, provider, and start/end times are present.Visit note, EVV, schedule, and billed discipline agree or differences are resolved.Authorization dates, discipline, frequency, and units cover the billed services.

5. OASIS and reporting

When OASIS applies, the completed assessment is transmitted within 30 days after completion.The current OASIS data set and CMS quality-reporting calendar were checked.The assessing clinician - not software - selected every final OASIS response.

6. Final billing readiness

Payer, period, orders, visits, authorization, EVV, and supporting records align.Every blocker is resolved or has a named owner and due date; unresolved packets are held.A human reviewer recorded the final decision and retained the worklist.
